The official forums for Median XL have a trade system that is similar to the old D2JSP system (you meet a player in a game, they give you an item, you transfer them forum gold on the website). My only real hesitation is that I'm starting to reach the point where it kind of feels like I need to start trading, which is clunky as heck. The content is enough to make Diablo 2 feel fresh again. Others are "dungeons" which are focused on a more mechanically interesting boss fight, some of which "lock" a short time after you zone in, to prevent death zerging. Some of them are "rifts" which are more of a traditional zone clearing type area, like a diablo 3 rift. A new tier of them becomes available every 5 levels starting at level 105. Level 115 means I just gained access to the third tier of end-game zones. I also have a bunch of ice elementals tanking for me. My secondary attack is "shatter the flesh," a sorceress corpse explosion skill that does cold damage. I use "stormfront" as my primary attack, a skill that shoots projectiles that electrify the ground. I'm currently level 115 with my sorceress.
The new boss / zone encounters are often novel and interesting, though perhaps not super well balanced. The annoying "inventory full of charms" thing from LoD is gone, because the only charms in the game are now the unique ones, and your inventory is big enough that even if you have them all, you still have plenty of space. The new endgame is a bunch of new zones, generally with a goal towards defeating a difficult boss, who will give a unique charm. If you have a nostalgic feeling about Diablo 2, and want to get back into it with tons of new content, Median XL Σ is where it is at. I personally am "fixing" this with keyboard macros to make it behave like a modern ARPG (mostly). The biggest annoyance to my mind, only having 2 buttons for active skills (with shortcuts to swap active skills around) is still there. The game still looks like it is from 2000, but some things have been done to modernize the interface.
The big gain with Σ is that he added a new engine. Median XL has been around for a long time, but Σ was released a few weeks ago. It offers thousands of new items, new skills for all classes, and multiple improvements to the Diablo II engine. The most popular Diablo II overhaul modification, Median XL is an action RPG with extensive endgame content, deep character customization and challenging gameplay.
